Improving access to lactation care by accepting some insurance plans.

Aetna Insurance
I am in-network with Aetna. If you have insurance through Aetna I will bill Aetna directly on your behalf. They provide coverage for up to 6 visits. This can be a combination of prenatal, initial consultants and follow up visits. Please book under "In-Network Insurance" in the online booking agent and provide all of your insurance information in your intake form.
Blue Cross
Blue Shield PPO, Anthem PPO, Humana PPO,
Provider Network
of America,
Multiplan
and Cigna PPO
Through a partnership with The Lactation Network I can bill OON for Blue Cross Blue Shield PPO, Anthem PPO, Humana PPO/POS, Multiplan and Cigna PPO. Click on the button below to check your coverage BEFORE your appointment. You must be approved prior to your appointment to be eligible for coverage. After approval book services under "Lactation Network" in the online booking agent.
Does your insurance card have this logo? If so you could be covered! Fill out the Lactation Network Verification Form above.
United Health Care
I am able to bill OON for United Health Care through The Lactation Network if you also have the Multiplan logo on your insurance card. Please verify coverage by filling out the Lactation Network Verification Form BEFORE you book your visit. If covered book your visit as "Lactation Network" in the online booking agent. If not covered please book as "self pay" and see All Other Plan section below.
Tricare
I am in-network with Tricare. I will bill Tricare directly on your behalf. They cover up to 6 visits. Because of Tricare's reimbursement rate these visits are office visits only. Please book under office visit and then choose "in-network insurance". If you have Tricare Prime you will need a referral from your primary physician to utilize my care. Tricare Select plans do not need a referral. Please contact me with questions.
Badger Care
I am happy to set up payment plans that fit your financial situation. Payment plans are set up as a monthly direct payment with a credit card on file. Please reach out if you would like to discuss your options.
All Other Plans
Please book using the "self pay" label in the booking widget. Payment is due at time of service and I will provide you with a Superbill to send to your insurance company for potential reimbursement. I also accept FSA and HSA cards, credit/debit and cash for payment.
The Affordable Care Act mandates coverage for lactation support so your insurance should cover your visits. HMO plans may require a referral from your health care provider in order to be reimbursed. You may also need a Network Gap Exemption for coverage by an out-of-network provider. Call your insurance company for more details.
